Wordpress by default organizes your uploads into folders labelled by the year and month.
So if you upload an image in January 2009 with a file name of sample-image.jpg to call that image using custom fields then you would need to use;
2009/01/sample-image.jpg
Which is a bit complicated if you forget which month you added an image and want to add the image to a post via a custom field.
To make things easier to remember, you can go into WP Admin/ Settings/ Miscellaneous and uncheck the “Organize my uploads into month- and year-based folders” which you can see in the image below unchecked from my Admin section

This makes Wordpress upload everything directly into the Uploads folder, rather than organizing it into date folders.
So referencing that same file again becomes a lot simpler, you would just need;
sample-image.jpg
So if you are using one of our themes, you may find this a simpler way of referencing your image files in the custom fields.Of course you may still want Wordpress to sort your images into folders, this will work perfectly well with our themes, but you should remember you will have to reference the folder AND the image when using custom field values.

I have this checked around 2 years. What happens if I remove it now? Will all my uploads be re-organized and the modified link will appear in my blogs that are already published?

No, your old uploads will stay in the same place, all future uploads will be moved around though. There is no right or wrong way of organizing it really, its all down to personal choice. I prefer keeping everything in one place and making it easy to get to.
